Getting started is easy. If you have received your member card, follow these steps to book your visit:
- Confirm Your Eligibility: Ensure you have been successfully enrolled in the CDCP and have your member information ready. If you have questions about how to apply, we are happy to share resources.
- Schedule Your Visit: Call us or book online. Mention that you are covered under the CDCP so we can prepare your file.
- Your Appointment: Bring your CDCP info to your appointment. Dr. My Le will perform a comprehensive exam and discuss a treatment plan tailored to your needs and your coverage.
- Direct Processing: We will submit the claim on your behalf. You will only be responsible for any co-payment (the portion not covered by the plan) or services not included in the CDCP schedule.
